Enrollment and Eligibility Forms & Information

FORMS and INFORMATION

To enroll in preschool, your child must be three or four years old on or before August 1st.

Our enrollment includes children whose families are income eligible and children with disabilities. Please call our office at 606-928-7110 for more information. 

Please be sure all areas are filled in. The application will then be processed in our office. You will have access to attach and send required documents also. 

Your child’s enrollment application must include a proof of income and a proof of birth.

PROOF OF INCOME FOR 2022- W-2, Income Tax Forms, child support, written employee statement, Declaration of Income Status

PROOF OF BIRTH:  a copy of the hospital Birth Certificate or Certified State Birth Certificate with the enrollment application may be temporarily submitted. However, a Certified State Birth Certificate must be submitted within 4 weeks. Forms to apply for the state birth certificate are also available at your child’s school.

The requested forms and documents listed below are included with the Enrollment Application, but they do not have to be turned in at the same time. The items below may be turned in at a later date, but must be filed BEFORE your child is placed in the classroom.
 
If it is determined that your child meets eligibility requirements for enrollment in either Kentucky Preschool or Head Start, the following documents must be on file before your child can be placed in a classroom.


A complete application for enrollment must have the following:

  • PROOF OF INCOME FOR 2022- W-2, Income Tax Forms, child support, written employee statement, Declaration of Income Status                    
  • State Birth Certificate (Or proof of Date of Birth until State Certificate can be obtained.)
  • SCHOOL PHYSICAL – Including Blood Pressure
  • HEMOGLOBIN/HEMATOCRIT—Inform your health care provider this is a Head Start Requirement.  If your child was in the WIC program at any time, you can obtain this information from the Health Department.
  • LEAD TOXICITY SCREENING – Inform your health care provider this is a Head Start Requirement.  If your child was in the WIC program at any time, you can obtain this information from the Health Department.
  • KENTUCKY IMMUNIZATION CERTIFICATE – We cannot accept a shot record.
  • The following documents must be on file with 60 days of your child’s enrollment.
  • COPY OF MEDICAL CARD
  • KENTUCKY EYE EXAM – Required by the state of Kentucky. This exam must be done by an optometrist or ophthalmologist, not your family doctor.
  • DENTAL EXAM –Must be done by a licensed dentist. A dental form is attached.
